The entire application takes a few minutes to complete.

The application can be submitted online. You can also print the completed application form and submit it by fax or by post. (fax or post submission method is not applicable on portable device version)

Terms and Conditions

1. Application and materials provided by the Department of Health are free of charge for health education purpose.

2. For online applications, each applicant (using the same email address) can submit 3 requests at maximum per day.

3. The Department of Health reserves the right to decide on the quantity of materials provided, depending on stock availability.

4. The Department of Health will contact the applicants through email / fax / telephone for matters including but not limited to collection time and quantities of items given. Please make sure you leave us a valid and correct email address, fax or telephone number.

5. The Department of Health will confirm the date to collect the requested health education materials within 5 working days after receiving the application by:


6. Application will be cancelled if the requested materials are not collected after 1 month, counting from the date of notification issued. Applicants will then have to resubmit their requests afresh.
